Office Policies

We make a strong effort to take care of you and your family in a timely manner. Some procedures naturally take longer than others. In order to treat you most efficiently, please keep in mind our standards when making a request.

To Request a Prescription Refill:

First, contact your pharmacy to see if you have any refills available. If not, contact the office (by portal, email, or phone) and request a refill on the medication. Please have the name of your medication, dose, and preferred pharmacy ready. We ask for 48 hours to process all prescription requests.

To Request your Test Results:

When we run a standard test, (i.e. blood test, MRI, CT scan, etc.) we try our best to have results back to you within 2-3 days. If you are expecting results, start by checking your Patient Portal. Results are posted here first with a call following. If the results are not posted on your portal, feel free to contact the office (by portal, email, or phone) for more information.

To Request a Form to be Filled Out by your Provider:

Please fax or bring a copy of the required form to the office. We ask for at least 7 days to complete any required forms or physician authorizations.

To Request a Referral:

If you are interested in a referral to a specialty clinic please contact our office (by portal, email, or phone) with your request. We ask for at least 7 days to process all referrals. Special exceptions may apply for urgent situations.

To Request a Copy of your Medical Records:

If you need your medical records transferred to another clinic, or a copy for your personal files, we require a signed records release. We ask for up to 2 weeks to process all records requests. We do not charge for records that are sent to other facilities/doctor’s offices. However, there is a charge if you are requesting the records for yourself. We follow the State of Colorado allowable fees
