Chronic Illness Management

The providers at Platte River Medical Clinic are committed to helping you manage your chronic illness and overall health. We send email reminders when you are due for your next appointment.

Chronic Illness Management with a Telehealth Visit

While we like to see you in our clinic, we can still help you manage chronic conditions through a telehealth visit. If you have Humana, you have an out-of-pocket amount that can be used to buy a thermometer, blood pressure monitor, and scale to help obtain your vitals for these visits. You’ll get the the same individualized, quality care you’ve come to trust from our clinic.

Chronic Illness We See Most Often

We treat a wide variety of common illness, but there are a few we see more than others. When needed, we work alongside specialists to promote your overall health.

Diabetes

Mental Health (including Anxiety and Depression)

Recommended Follow-Up Appointments: Every 6 months

Mental health is an important aspect of our overall health and well being. Mental illness is common with 1 in 5 American adults experiencing a mental illness at some point in their lives. According to the Centers for Disease Control and Prevention, a number of factors can cause mental illness including biological factors, long-lasting health conditions, and stressful or traumatic life events. Schedule an appointment with one of our providers to determine your wellness plan.

Hypothyroidism

Recommended Follow-Up Appointments: Annually

Your thyroid has influence over every cell in your body. When your thyroid is under active, your body does not get enough of the thyroid hormone, which may cause you to feel fatigued, experience weight gain, and a slowed heart rate. See one of our providers to determine the best therapy for your under-active thyroid.

Hyperlipidemia (High Cholesterol)

Recommended Follow-Up Appointments: Annually

There are more than 102 million American adults with high cholesterol. There are no symptoms, so getting your cholesterol tested is the best way to determine if you have hyperlipidemia. Risk factors include a poor diet, lack of exercise, smoking, age, and diabetes among others. See one of our providers to determine the best treatment plan for you or to get your cholesterol checked today.
